The Kendall Couty Coroner's Office encourages hospice and palliative care agencies to pre-register all anticipated home deaths to ensure a smooth reporting process upon the patient's passing.  By completing the pre-registration, you assist our staff in verifying the information that will be collected at the time of death (such as demographic information) as well as greatly minimizing the chances of delays or follow up calls from an investigator when the death occurs. To make this process easier for your staff, we have created an online form below that can be submitted electronically 24 hours-a-day.

NOTE: Pre-registration of a patient does not relieve the medical professional of their responsibility to notify the coroner's office upon pronouncement of death. The coroner requires ALL deaths occurring within Kendall County to be reported immediately by calling (630)553-4200.
